Cancer Transitions is a free, 2-hour, six-week workshop designed to help cancer survivors make the transition from active treatment to post-treatment care.
Participants learn safe exercises from a physical therapist who specializes in cancer exercise. Physicians, social workers, nutritionists and nurses also discuss topics and tips for stress management, transitioning back to primary care follow up, and finding a new normal after treatment ends.
The program recommends that patients be one month up to 2 years out of active treatment.
BEGINNING: June 13, 2012 (plus five consecutive Wednesday evenings following).
Location: Poudre Valley Hospital, lower level. Sessions begin with 30 minutes of fun, informative and supervised exercise in the PVH Cardio-Pulmonary fitness room, with the remaining educational and group session in Mortensen conference room.
Light lunch and beverages will be provided. There is no cost to participate.
