Education about your medicines

During your admission, our staff will ask you what medicines you routinely take. This is important information for your nurse and doctor. Your doctor will decide which medicines you will continue to use during your hospital stay.

We will also review this list the day of your discharge.

Completing this medication form before your stay can help the process. The form allows you to list medications you're taking, allergies to medications, and provide emergency contact information.

You will receive handouts on any new medicines. This will have information on any interactions with your existing medicines. Your nurse will review this with you. We encourage you to ask any questions.
