General heart health resource opportunities
Stong Women, Healthy Hearts - Sponsored by PVHS' Aspen Club, this 12-week program is designed for women aged 40+ to actively participate in activities and education that lead to a healthy lifestyle and reduced risk for heart disease. For one hour, twice per week, participants learn about and participated in moderate intensity exercise, stress management, weight loss and maintenance, blood pressure checks, healthy cooking and proper nutrition. Enhance Wellness - This Aspen Club sponsored national, award-winning health and fitness program is designed especially for people 50+ dealing with one or more chronic conditions. The six-month, individualized program helps you identify your strengths and health risks and choose realistic goals to improve your health and well being. The program addresses overall wellness, fitness classes and consultations and disease self-management classes. The program features one-on-one support and guidance from a community health nurse, fitness professional, nutrition specialist and peer-coach. Healthy Hearts ClubTM - This outreach heart health screening and preventive education program is offered to many adults and elementary and high school students in Northern Colorado.

- The Health District of Northern Larimer County offers adult blood screenings low-cost or no-cost many times throughout the year.
Fitness and weight management opportunities
Aspen Club fitness programs - For people aged 50+, the Aspen Club offers a variety of exercise classes, including yoga, core strength, beginning exercise, strong women/strong bones and Zumba Gold. Walking groups meet in Loveland and Fort Collins May through October. One-on-one exercise / fitness consultations with an exercise physiologist will help you start or continue safe and effective exercising, share tips for cardiovascular, strength and flexibility improvement (member's fee: $25).
A comprehensive fitness assessment measures flexibility, upper and lower body strength, cardiovascular fitness, body circumference and body composition. PVHS conditioning classes - Offered in the cardiac rehabilitation areas of Harmony campus in Fort Collins, Medical Center of the Rockies in Loveland, conditioning classes are open to adults seeking to improve or maintain their health through excercise in this safe, modern and friendly environment. Poudre Valley Medical Fitness - Through clinical integration, this Windsor-based wellness center focuses on the prevention and treatment of lifestyle-related disease, illness, and injury through regular participation in physical activity, disease management and health education.
Aspen Club weight minders group - This group meets twice a month to discuss different

Blood pressure resource opportunities

The Nurse Is In - This Aspen Club sponsored program is offered to members in Loveland, Windsor and Fort Collins for people who wish to get their blood pressure checked at no cost and talk to a licensed nurse.

Ethnic-based heart health education opportunities
Ama Tu Vida! is a education program targeting the Latino population of Weld County, Colo. The goal of the program is to decrease the incidence of cardiovascular disease (CVD) among the target population by reducing the risk factors associated with CVD, including diabetes, high blood pressure, high cholesterol, and obesity.
