Question: How do I get a copy of my images and report?
Answer: Please call our Archive Center at Poudre Valley Hospital 970.495.8604 or Medical Center of the Rockies 970.624.1400. You will be asked to sign a release of information consent and provide identification at the time you pick up your information.

Answer: Yes, you may have someone else pick up your information. At the time you call for your imaging record, you will be asked to provide the name and means of identification for the person who will pick up your imaging information. This could be a birth date or social security number. This person will then need to provide our staff with that same information at the time of the pick up.

Question: Why did I receive two bills?
Answer: You will receive two separate bills. One is for the technical service provided by the hospital. The second bill is the professional fee from the Radiologist who interpreted the images.
